Access to Health Records
Patients are able to request copies of their medical records. This can be done by submitting a Chat Doc request or by sending in a written request to the practice. The original medical records may not be removed from the practice premises.
Access to medical records for people outside the health care team (or those who are involved in the patient's clinical care) is only given with the patient's written consent.
We are able to send your records via a secure e-mail or patients are able to collect printed copies if necessary.
Requests will be assessed accordingly but please note this process can take up to 30 working days.
