New Patient Registration - Adults

Registering with our Practice

We now offer Online Registration with the Practice - start the registration process.

We prefer to meet all our prospective patients even if they have no current medical problems. New patients are welcome to register between 8.30am and 6.30pm Monday to Friday. Patients are requested to bring in two proofs of address eg utility bill, bank statement etc (if you require internet access, one of these would need to be photo ID). If you are unable to provide any documentation this will not discriminate you from registering with the practice if you live within our boundary area. Patients will then be allocated a GP. You will normally be asked to complete a registration form. This can be download in advance.

Medication

If you are taking repeat medication medication, please provide us with a copy of a repeat slip of your medication or a print out from your previous surgery. We cannot accept hand written lists.

New Patient Appointment

All new patients aged over the age of 18, once registered, are requested to book a new patient health check with our Health Care Assistant.

As our Practice will not receive your previous medical records for several weeks, it is important that we obtain details of family and medical history, to enable us to provide necessary and appropriate care.

Disabled/Wheelchair Access

The Practice building complies with the current regulations for wheelchair access, with consulting rooms on the ground floor. Should there be the need for a visit to the upper floor a lift is available to patients. Disabled toilets are also available for the use of patients.

Temporary Residents

If you need to see a doctor but are away from home or not registered, you can receive emergency treatment from any GP practice. You will be asked to complete certain forms as a record of your visit to the doctor and this will be forwarded to your registered doctor.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact us. Please note GP practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.
